Medicare funded care plans are available for eligible patients suffering a chronic illness of more than 6 months. This involves a treatment plan made for you by your Doctor and entitles you to five bulk billed visits with a relevant health professional.
Please discuss any of the following type of disorders or symptoms with your GP.
These care plans may include: Diabetes, asthma, skeletal problems, chronic heart conditions, back pain, psychological disorders (referral to a Psychologist can be for any Psychological disorder and you can have up to 12 visits per year. Consultations are not fulfilled but there is a substantial Medical Rebate).
FREE HEALTHCARE ASSESSMENTS available for over 75's. Our Registered Nurse will perform Health Care Assessments in the privacy of your home at your Doctor's request.
FREE HEALTH CHECK available for all patients 45-49.
